Canon 20D with the 580EX speedlite..underexposed?

I have the Canon 20D and I use the 580EX speedlite and I have been finding lately that my pictures come out horribly underexposed while using the flash...any ideas?

A sample image would help, but typical things to check include:

(a) make sure Flash Exposure Compensation on the camera and on the speedlight are set to "0". FEC set on the speedlight will override that set on the camera.

(b) make sure the speedlight is set for E-TTL and not Manual output level.

(c) the E-TTL flash exposure system gives greatest weight to the meter segments around the active focus sensor. If you use center focus/recompose/shoot then the meter segments around the active focus sensor (center) will not be on the subject when the flash fires, resulting in exposure error. If you are going to focus/recompose/shoot, then you need to use Flash Exposure Lock at the time you lock focus.
