Sue- the main thing is the white background & bright enough lights. You want the lights to blow out all the light details (like the skin) but retain the detail in the darks (like the eyes, the nose & lips).I have found it is difficult to do without 4 lights. 2 for the background, (that actually ends up acting as a reflector once it's lit) & 2 large foreground lights. By large I mean, either large soft-boxes or large light panels in front of the flashes. Either will work.
I did try this once with 3 lights & 2 large light panels opposite 1 light that was a large soft-box, but I still needed the 2 background lights. It did work well.
It's also very important that you shoot it in the RAW format. This way you can adjust the "brightness" & "shadow" to achieve the look you want. You can also over-expose it a little more, if you need too.
