I worked on this image a long time ago, Eric, so my memory isn't very good, but here goes:
1. Open your image in Photoshop and make a duplicate layer of it.
2. Below the duplicate layer create an empty layer and fill it with black.
3. On the black filled layer select 'Effects - Gradient Overlay' and choose the 'Soft Stripes' gradient overlay that looks like black window blinds.
4. Adjust the effect to give the appearance of window blind shadows.
5. Crop the image layer to look like an apple. Use the same effect used on the black layer to give the image layer the 'Soft Stripe' effect.
6. Use the 'Spherize' effect to give the image layer its distorted look.
7. Add a 'Stroke' effect to the image layer to give it a thick glass edge look.
8. Use 'Render - Lighting Effects' to adjust the lighting to make it look as if the light is coming through the window.
