How do I get a model release form? Do I have to make one my self or are there places where you can get them?

  I would highly highly recommend using getty's model release. You can download the model,minor,and property release in all major languages at the following site.

They have full sheets and half sheet print outs. You can take them to your local Kinkos and have them padded. Currently at Kinkos you can have a set of 100 padded for about $8.00

If you use the following model release you will cover many more areas of concern than the release stated above. Also, if you intend to sometime in your future get accepted into gettys or corbis it is ESSENTIAL that you use this agreement, if you do not then gettys will not defend you against liablity or lawsuit and very well might not accept your images.
