Why are my pictures graini

The blacks in my picture do not appear as a sold balck but are grani. why is this? I am using ISO400 film at F.4 60, with internal flash; indoor pictures. does a grani picture mean the picture is over exposed or under exposed. And how sould I go about reducing this graininess.

Well yes I know to check for those things. The question is: Is a grani picture the result of a picture being over exposed or under exposed.
