John,I actually had a total of 960mm when you combine the 2x Extender and add-in the Canon 10D 1.6x smaller cmos sensor.
But back to the question you asked. I have been taking sharp handheld images like this with the combination of alot of patience and learning my own style and with Canon's(IS)ImageStabilazation which is GOD Send. If I am hand holding I alway make sure there is enought light to make my shutter speed above 125th/sec.
