In answer to your questions, I use Canon EF 100-400mm f/4.5-5.6 L IS USM for my bird shots.
My blind is the kitchen window. Birds don't seem to mind, unless I appear suddenly. I know when they will come and when I have the best light. Sometimes I will leave the camera there (always on the tripod) and come to the kitchen every now and then to see who is there, or when I hear a bird racket going on. The main thing is not to trip over it :-)
I shoot through glass, at an angle, with and without a flashlight.
Window must be very clean for good shots.
The advantage I have is that 'my' birds don't have any other water available close by, it's very hot, so they love the birdbath. In fact it's a life saver for them.
