Film that comes back clear

I got a roll of film back withthe 1st 6 pictures perfectly exposed. The next 8 frames were clear then 1 frame fine and the rest of the roll was clear again.

All the photos were taken int he same situation and I didn't change any camera settings.

What would cause the film to come back clear (with a few little black spots)?

Underexposure. Not enough light reaching the film.

Could be cause by any number of things, including:
(a) shooting with the lens cap on
(b) flash not firing
(c) using too fast a shutter speed and/or too small an aperture setting
(d) faulty shutter not opening or faulty lens aperture closing down completely.
