mpix.com offers calendars---I haven't made one before with them (or anyone) but I've used mpix for prints and have been happy with the results.
I'm sure there are many other online places that offer photo calendars (Clarks, Kodak, etc.) as well as a few walk-in places (WalMart, Target) to consider as well.

Lulu.com and Vistaprint.com both let you create your own calendars. I have used both. Check them both out.

Like Dennis, I've used Lulu.com...it's very nice and easy to upload your photos and view previews.

WalMart has an easy machine at the photo place. You select your layout, words and add the photo from your disk or USB and then click print. Easy as 1,2,3

I've used shutterfly in the past, but they do have their logo on the back so it depends on how you feel about that one as to it you want to go elsewhere. I've used mpix for prints too with good results but haven't tried to make a calendar w/ them.
