Stock Sheets for Publisher Submission


I am currently trying to pull together a themed portfolio for submission to a publisher looking for ideas for books, calendars, etc. I received the submission guidelines and request through a stock photo site I belong to. Besides the photos, they are looking for tear sheets (I only have one) and stock sheets. I know that stock sheets are to show them your photos are selling, but what is a general format for these? All my photos have sold on-line through a stock site. I only know the actual uses for two of them. I have the date sold, the amount sold for, and (for two of them) the usage. For the publisher's stock sheet, what do I include or how do I set it up? Do I place a thumbnail next to the image title with a date sold and usage if applicable? Also, when submitting the photos, is it best to place them in an album/portfolio for submission rather than just loose in an envelope or do they find portfolios too cumbersome to keep around. I just want to make a good impression, look like a pro and NOT have my stuff end up in the junk mail trash can;-) Right now, I have some small portfolios (hold 8.5x11" images) from Mr. Art that also fold up into an easel for display. I was thinking of sending one of those. It's simple black, not too thick, and lightweight. I'd greatly appreciate any advice anyone could give on this subject.
