Release Forms


I shot a family reunion a month or so ago and one shot of an individual family came out really nice and I'd like to use it for my Portfolio.

I know I need to get permission to use it, but I have a couple of questions.

1) This is a family shot. The parents are the only ones in the shot that are over 18. (There are two teenagers though) Do I need a general family release signed by the parents or "should I" also get the teens to sign?

2) With this release, can I use this picture forever? Is this something that is contract specific?

3) Is there a difference between the Release Form that I'm looking for (or looking to create) and a model release?

4) Where can I find an example? Good books? Lawyer? Web Sites?
