How or can you hand paint pixel by pixel


I have seen some beautiful hand painted work, how do you do it. I have 7.0 photoshop.

  Some pictures are done by just adding an effect to a photo with filters in photoshop or correl paint.
Others are done where somebody uses a mouse and traces a layer on top of a photo.
And it can be done with an effect done to a cut/paste image with a freehand painted background. Or everything freehand.
Most of the ones seen start out as a photo, then the program changes it into the paint appearance. Correl seems to have a more paintbrush effect than photoshop. With photoshop, using the watercolor filter, the affect isn't always that apparent because you retain so much detail that from medium distances it can look like nothing was done at all.
Correl comes out more abstract, with better brush stroke effects.
If you wanted to do something totally freehand, you can with the different size brush tools in photoshop. If a mouse felt awkward, there is a thing that you can hold in your hand like a pencil and do drawing like your screen were paper. But I don't know what it's called.
